First and foremost, find your PARENT PACKET here! This will provide helpful information in preparing for camp this summer.
The following forms are accessible to complete digitally on your camper's Ultra Camp account and should be easily submitted through the online system. If you are unable to process the forms online, please utilize the forms found below and bring them with you at the time of check-in.

Log onto your child's registration account online and complete the medical history form using the Ultra Camp system. If online submission isn't possible, please click the link above to download and complete the paper form.
Note: There is a portion of the form which requires review by your family physician.
Medical information will only be accessed at camp by administrative staff or the camp nurse. Counselors will be given confidential reports as necessary to alert them to the needs of your child.
